Population health can be defined as the application of public health principles to health care. It is how we improve the health of our patients and their communities- promoting wellness and prevention while mitigating the impact of chronic disease. At first glance, this neat and concise definition can create the illusion that population health is a simplistic topic. However, on the contrary, population health is a complex, multidimensional focus in healthcare, evidenced by the numerous challenges that manifest as barriers to the advancement of population health initiatives today. It is because of the persistence of these very challenges that physician leaders must develop enhanced knowledge and competence pertaining to the principles, goals, and strategies that advance population health initiatives.

A Journey Through Population Health is designed to aid physician leaders in developing a more comprehensive and actionable knowledge base around population health. Using video lectures, self-reflective inquiry, and interactive learning activities, this course will illuminate your awareness of both the complexity and scope of population health, equipping you with the perspectives and approaches to effectively lead in the healthcare setting with a deepened awareness of the impacts of population health factors on patient care. We will address the challenges faced and solutions created by 21st century healthcare providers.  We will also discuss how prevention and quality outcomes are linked to an organization’s bottom line, leading to an exploration of the latest critical data, pivotal studies, and key viewpoints on population health.

Course Objectives

At the completion of this course, participants should be able to:

  • Identify the key stakeholders in the U.S. health care system and the complex interrelationships that influence population health outcomes.
  • Explore the intricacies of the population health paradigm, including strategies for improving tomorrow's population health management.
  • Examine the economic concept of risk and how the application of risk management theory can alter population health outcomes..
  • Review the application of data analytics and performance measurement tools for improving success. .
  • Explain the role of quality improvement in the context of population health management.
  • Describe how a population health perspective can affect health care priorities in the areas of prevention, public health and health policy.
  • Identify the significance of managing dimensions of wellness in longterm human health.

Method of Participation

To complete this program successfully and claim credit, participants must follow these steps:

  • Read the educational objectives provided on the course summary page.
  • Attend the sessions or watch all the video lectures for this course (if applicable).
  • Read any course materials and complete any required activities.
  • Complete the course evaluation and post-test via your dashboard on the Association website. Successful completion of a given activity’s post-test requires a minimum 75% passing score prior to claiming CME credit, with three attempts allowed.
  • CME certificates will be available for printing after claiming CME credit.
  • Estimated time for course completion is equivalent to the CME credit hours.

Cancellation Policy

  • Refunds are available for online courses if the refund is requested within a 72-hour period after purchase and no course materials have been accessed.
  • If the course is accessed at any time, no cancellation or refund will be allowed.
  • If registrations are cancelled within 72 hours of purchase and no access of course material has occurred, then a full refund less a $100 processing fee will be applied.
  • Once 72 hours have passed after purchase, no cancellations or refunds will be allowed.
