I understand and agree that as part of participating in AAPL’s Leading Innovation in Healthcare Organizations (“Event”) that all or some portions of the Event may be recorded by AAPL for the limited use of making such Event available to other participants of the Event for review for no more than ninety (90) days following the completion of the Event.  

As such, I understand and agree that any images, photographs and/or video and audio recordings of me captured during the recording of the Event (“Images”) may be placed on the Internet following the completion of the Event and all such Images are and shall remain the sole property of AAPL.  I also understand and agree that I may be identified by name and/or title in printed, Internet or broadcast information that might accompany the Images.  I understand and agree that I will not have an opportunity to review the final Images before they are published or distributed.  

I agree that I will not record, copy, reprint, distribute, provide access to and/or make available to anyone else any of the materials and/or content recorded at the Event without AAPL’s express written permission and I will access the Event materials and content solely in connection with my participation in the Event.  I release, acquit and forever discharge AAPL, and its current and former trustees, agents, officers, employees, and affiliates, from any and all claims, demands, rights, promises, damages and liabilities arising out of or in connection with the use or distribution of the Images, including but not limited to any claims for invasion of privacy, appropriation of likeness or defamation.

I acknowledge that my participation in the Event and video portions of the Event is completely voluntary, as is my consent to this release, and by completing the required registration materials for the Event I have read and agree to the terms of this release.
